Access to your information
Under the new General Data Protection Regulation (GDPR) 2018 everybody has the right to see, or have a copy, of data we hold that can identify you, with some exceptions. You do not need to give a reason to see your data.
 
If you want to access your data you must make the request in writing. Under special circumstances, some information may be withheld. If you wish to have a copy of the information we hold about you the following staff procedure details how this can be achieved:
 
Access to Medical Records processing
 
Patients have a legal right of access to their health records, which North Kensington Medical Centre is happy to support.  The following procedure has been agreed to make this easy and straightforward.
 
Patients may access their records in one of two ways:
 
  1. To View medical records:
 
  • Patients should request access to their notes in writing. This can be handed in at reception, posted to the practice or e-mailed to (?) The Reception Team (RT) will pass notes and computerised print out of records to the usual doctor informing them that the patient requests to see their medical records.
  • The usual doctor will check there are no inappropriate references to third parties or information which may be damaging to the patients' health.
  • The doctor will instruct the RT if it is necessary to withhold any information.
  • RT will contact the patient to arrange for them to come and view the records in the presence of a member of the admin team. The records must not be taken off the premises or altered in any way.
  • If concerns are raised about the content of the records, an appointment will be arranged with their usual doctor.
 
  1. To obtain a copy of the medical records
 
  • Patients should request a copy of their notes in writing. This can be handed in at reception, posted to the practice or e-mailed to (?)
  • The Reception Team (RT) will pass a copy of the notes and computerised print out of records to the usual doctor informing them that the patient requests a copy of their medical records.
  • The usual doctor will check there are no inappropriate references to third parties or information which may be damaging to the patients' health.
  • The doctor will instruct the RT if it is necessary to withhold any information .
  • The Reception Team will contact the patient to advise that the notes are ready for collection. In line with GDPR as from 25.05.18 no charge will be made for providing copy records
  • If concerns are raised about the content of the records, an appointment will be arranged with their usual doctor.
 
Both the above will be completed within 20 days of the request being received
